The Saskatchewan Medication Assessment Program (SMAP) gives you the opportunity to meet one-on-one with your Hill Avenue Drugs pharmacist to review all of your medications and ensure you’re getting the best possible results.
If you’ve ever felt unsure about your medications, struggled to keep track of them, or questioned whether they’re working as they should, a medication review can help.
Do I Need a Medication Review?
You may benefit from a review if you:
Take multiple medications
Have questions about how or when to take them
Are experiencing side effects
Have recently had changes to your prescriptions
Want to better understand your treatment plan
Am I Eligible?
You may be eligible for a funded medication review if you:
- Are 60 or older
- Take 5 or more long-term medications
- Take blood thinners (such as warfarin)
- Take a medication listed on the Beers Criteria
- Are experiencing medication-related side effects
If you’ve recently been discharged from the hospital or are starting a new medication (like insulin), a review can be especially helpful.
What Happens During a Medication Review?
Your pharmacist will:
- Review all your medications (prescription and non-prescription)
- Explain what each medication is for
- Ensure you’re taking them correctly
- Identify any potential interactions or issues
- Recommend adjustments if needed
The goal is simple — to help you feel confident and get the best results from your medications.
What Should I Bring?
Please bring all medications you are currently taking, including:
- Prescriptions
- Over-the-counter medications
- Vitamins
- Natural health products
Do I Need a Doctor’s Referral?
No referral is required. While your healthcare provider may recommend a review, you can book directly with your pharmacist.
Will This Improve My Health?
A medication review helps you better understand your medications so you can make informed decisions about your health. It may identify opportunities to:
- Reduce side effects
- Improve effectiveness
- Simplify your medication routine
What If I’m Not Eligible?
Even if you don’t qualify for a funded review, our pharmacists are always available to answer your questions and provide guidance.
Booking for a Family Member
If you’re helping a family member, ensure they are interested and provide consent before
booking an appointment on their behalf.
Is There a Cost?
Medication reviews may be covered through the Saskatchewan Drug Plan. Ask our team to see if you qualify.
